Insurance

Family Practice Associates, accepts most insurance plans.
Please contact our office at 270-352-4601 or call the customer service number on the back of your insurance card to verify we do participate with your insurance plan.

Our office will file insurance claims for you. In the event that the claim is denied due to non-coverage under your insurance policy, pre-existing conditions, out of network providers, non-covered medical providers etc. You will receive a bill for unpaid services.

CO-PAYS, DEDUCTIBLES AND PATIENT PERCENTAGES

All Co-pays, Deductibles and percentages are due at the time of service. We accept cash, check and or credit/debit cards.

SELF-PAY PATIENTS

Self-pay patients are patients with no insurance coverage.
You will be required to pay for services prior to being seen by our providers.
There is a 40% discount for self-pay patients.


AUTOMOBILE ACCIDENTS

We do file automobile insurance for new injuries however, you will need to bring with you billing address, phone number, claim number and the name of your claims adjuster assigned to your accident. If you do not have this information you will be required to pay for services before being seen.

WORKERS COMPENSATION

Our office does see new workers compensation injuries, You will need to bring with you billing address, phone number, claim number and name of your claims adjuster assigned to your case.  If you do not have this information you will be required to pay for services before being seen.

LABORATORY AND RADIOLOGY SERVICES

We do have providers on site who render these services as a convenience to our patients. However both the laboratory and radiology departments bill and file insurance claims for patients separately from our office.

Patients do have the right to have these services rendered at any facility they may choose.

REGISTRATION AND PATIENT INFORMATION UPDATES

Patients will on their initial visit be asked to fill out office registration information on themselves and any family members who are new to our practice.

Patients will be asked for insurance cards and driver licenses on a routine basis so please bring these with you to every visit. Patients will also be asked to update a new patient registration packet every year.
